First (1st) Quarter 2017 FHV Evaluation Data Reporting

The deadline for reporting Family Home Visiting (FHV) data to the Minnesota Department of Health (MDH) for the first quarter of 2017 is Monday, April 10, 2017.  For a complete summary of the FHV Reporting Requirements, see the FHV Reporting Requirements 2017 document for guidance.   

If you need a FHVRES or MDH File Transfer Site account, or have any other questions or concerns about reporting for Quarter 1 2017, please contact Health.FHVdata@state.mn.us.

MIECHV Grantee Announcements

New 2017 MIECHV Quarterly Data Collection Form & Quarter 1 Due Date

Please report your first quarter Maternal, Infant and Early Childhood Home Visiting (MIECHV) quarterly data using this New PDF form. The attached flowchart provides additional guidance related to MIECHV quarterly data reporting. MDH is developing a web-based reporting tool for quarterly data collection.  Until we transition to the new tool, we will continue collecting the data using this PDF form.

The deadline for reporting MIECHV quarterly data to MDH for the first quarter of 2017 is Friday, April 28, 2017.

Upcoming Continuous Quality Improvement (CQI) Activities for MIECHV Grantees

An e-mail was recently sent to MIECHV Grantees participating in the CQI Learning Collaborative regarding the following upcoming CQI activities:

  • 1st Learning Collaborative Webinar-  Remember to mark your calendar! The first webinar of the Learning Collaborative is Thursday, April 20th, 1:00-2:00 p.m. 
  • CQI Reporting Reminder- Your March data is due by April 10th.  Data collection for this month will include your March numbers for Measures 1 and 2 of the screening areas you are working on, as well as a completed PDSA cycle.

Parent Survey Consultation Session This Week!

April 5, 2017 from 9:30 - 11:30 AM (CST) – West Saint Paul, MN

The next Parent Survey consultation session with Emily Wharton, certified Parent Survey trainer, will occur tomorrow, April 5, 2017 from 9:30 – 11:30 AM. The Parent Survey Consultation Sessions are intended to provide participants with greater knowledge and experience using Parent Survey through: 1) information on inter-rater reliability; 2) de-identified case scenarios; 3) step-by-step survey scoring; and 4) feedback about scoring at practice sessions.  

Twin cities metro area participants are highly encouraged to attend in-person at the Dakota County Northern Service Center (1 Mendota Rd W, West St Paul, MN 55118).
