SUBJECT:     Minnesota Providers on the National Disqualified List

 

 

In an effort to provide Family Day Care Home (FDCH) sponsoring organizations with timely information about terminated providers, the Minnesota Department of Education-Food and Nutrition Service (FNS) is sending an updated list of terminated providers each month. The file is named with the corresponding month and year in which it was extracted from the FNS tracking log of seriously deficient providers. This list only includes providers that were terminated from Child and Adult Care Food Program by FDCH sponsors in Minnesota.

As part of the provider application process, sponsor organizations must ensure that a provider is not on the national disqualified list, has not been determined ineligible to participate in a publicly funded program by reason of violating that program’s requirements, nor has had a conviction within the past seven years for any activity that indicated a lack of business integrity before application approval.
