HMIS Software Transition and Reporting Update
In 2023, the Minnesota Homeless Management Information System (HMIS) Governing Board and Minnesota’s HMIS administrator, Institute for Community Alliances (ICA) led a community-engaged software review, resulting in the decision to change software vendors and transition to a new system, called ClientTrack. The new system went live on August 2, 2024, and ICA has been adding HMIS users to the new live site in a phased approach. Please see the Go-Live Status Tracker for daily updates. At this time, the reports that Minnesota Housing would require for submission for the reporting period of May 1, 2024, through September 30, 2024, are not fully functional.
HMIS Reporting for Long Term Homeless and High Priority Homeless Projects
HMIS report submission for this reporting period is not required for projects with capital financing from Minnesota Housing that include units set aside to serve Long Term Homeless (LTH)/High Priority Homeless (HPH) households and Housing Trust Fund (HTF) Program Rental Assistance administrators.
Instead, projects with LTH/HPH units and HTF Program Rental Assistance administrators are expected to use this reporting period to make sure they are adequately set up in ClientTrack, including making sure that their project’s end users have completed training, have logged in, and have resumed data entry.
Regular report submission will resume during the next reporting period, inclusive of data entry covering October 1, 2024, through March 31, 2025, with reports due on May 1, 2025.
High Priority Homeless Referral and Documentation Transition Expectations
Coordinated Entry Priority List managers are able to make housing referrals in ClientTrack, and referrals for vacancies in HPH units are still expected to be filled through Coordinated Entry during this transition.
The HPH report can be ran retroactively once it’s available in ClientTrack. In the meantime, the Coordinated Entry Priority List manager can complete the HPH Eligibility Confirmation Form for Households Not Entered in HMIS, or verify by email that the household was referred by Coordinated Entry and is considered eligible as High Priority Homeless.
