The Minnesota Department of Health (MDH) is seeking applications from qualified responders to provide harm reduction, health, and housing hubs. The purpose of these grants is to reduce fatal overdoses through coordinated housing, harm reduction, social and medical services. Grantees will engage with people experiencing homelessness at high risk of overdose and offer services to assist with housing stability, harm reduction, and health care. All hubs will need to include a combination of housing and harm reduction services, but grantees have flexibility in determining where and how to offer services. MDH’s goal is to support organizations with the necessary experiences and expertise to effectively serve people most affected by homelessness and drug-related deaths, specifically African American and American Indian communities.
Eligible applicants include community-based organizations, community health centers, federally qualified health centers, tribal governments, faith-based organizations, educational institutions, county governments/community health boards, and any other public or private non-profit, not-for-profit, and for-profit organizations. Applicants are encouraged to apply for funding individually or in collaboration with other qualified and eligible entities to develop a comprehensive proposal. Collaborative proposals must designate a lead applicant.
An optional information session will be held on Thursday, January 11 at 2:00 p.m. A Letter of Intent to apply is due by Tuesday, January 16, 2024 at 4:30 p.m. The RFP deadline is Wednesday, January 31, 2024 at 5:30 p.m. Learn more and view the full RFP details at the Harm Reduction, Health, and Housing Hubs website.
The Minnesota Department of Human Services (DHS), through its Behavioral Health Division (BHD), seeks proposals from qualified responders to provide Projects for Assistance in Transition from Homelessness (PATH) homeless outreach services and Housing with Supports for Adults with Serious Mental Illness grant program (HSASMI) housing services.
The primary goals of both the PATH and HSASMI programs are to prevent or end homelessness for adults with serious mental illness or co-occurring substance use disorder who face high barriers to obtaining and maintaining housing and to assure that people transition from homelessness to housing. Proposals must also address one or more of the priorities of equity focused services, peer delivered services, ending veteran homelessness, or linking people to targeted housing rental assistance programs.
Responders may apply for PATH, HSASMI or both programs. The PATH program supports the delivery of outreach, case management and related services for people experiencing homelessness. The HSASMI program supports services for people to obtain and retain lease-based permanent supportive housing in single to scattered site housing models.
Review all of the details for this RFP on DHS grants and contracts page. Proposals are due by 4 p.m. CST on January 31, 2024. A Responders’ Conference will be held on January 5, 2024, from 1:00 to 2:30 p.m. CST via Webex and information to join can be found on page 19 of the RFP document.
