At the end of October, the Council, in partnership with our justice consultants with lived experience of homelessness, committed to Crossroads to Justice, the state’s plan to create new pathways to housing, racial and health justice for people facing homelessness. This plan will be the living guide for the state’s work on housing and homelessness over the next four years. Thank you for your partnership, guidance, and engagement as we developed this plan. The hard work to implement the plan is beginning and we will continue to seek your wisdom and guidance throughout the implementation.
As our team reflects on all the work that has happened in 2023 to develop this plan and advocate for a historic investment of resources, we recognize the work you do every day with so much kindness and compassion to support people facing homelessness across our state. There is so much work yet to do to ensure that all our relatives have a safe and stable home. Thank you for your relentless commitment to justice. We look forward to continuing this work together in 2024.

The Minnesota Department of Human Services (DHS), through its Behavioral Health Division (BHD), seeks proposals from qualified responders to provide Projects for Assistance in Transition from Homelessness (PATH) homeless outreach services and Housing with Supports for Adults with Serious Mental Illness grant program (HSASMI) housing services.
The primary goals of both the PATH and HSASMI programs are to prevent or end homelessness for adults with serious mental illness or co-occurring substance use disorder who face high barriers to obtaining and maintaining housing and to assure that people transition from homelessness to housing. Proposals must also address one or more of the priorities of equity focused services, peer delivered services, ending veteran homelessness, or linking people to targeted housing rental assistance programs.
Responders may apply for PATH, HSASMI or both programs. The PATH program supports the delivery of outreach, case management and related services for people experiencing homelessness. The HSASMI program supports services for people to obtain and retain lease-based permanent supportive housing in single to scattered site housing models.
Review all of the details for this RFP on DHS grants and contracts page. Proposals are due by 4 p.m. CST on January 31, 2024. A Responders’ Conference will be held on January 5, 2024, from 1:00 to 2:30 p.m. CST via Webex and information to join can be found on page 19 of the RFP document.
