Watch your mail in January 2026. Hennepin Health will send you a new member ID card and information about your health benefits. This will include where to go for care, when to go, and how to earn gift cards through the Hennepin Health rewards program.
Also, some Hennepin Health members will get a letter from the Minnesota Department of Human Services (DHS). It will invite you to take a survey called CAHPS®. This survey asks about your experience with Hennepin Health and helps us improve. If you get the survey, we hope you’ll take a few minutes to fill it out.
Flu and COVID-19 shots help keep you, your family, and your community healthy. These vaccines are free with your Hennepin Health insurance.
Now is the time to get your flu and COVID-19 shots if you haven’t already. They are safe and fully covered by your health plan ─ there’s no cost to you.
You can get vaccinated at many clinics and pharmacies. Check this list of pharmacies or visit www.health.state.mn.us/people/immunize/basics/vaxfinder.html for more locations.

You also are eligible for free at-home COVID-19 tests. Just ask your primary care provider to prescribe an at-home COVID-19 test and pick it up at your pharmacy.
Taking care of your teeth is just as important as taking care of the rest of your body. Going to the dentist helps keep your whole body healthy. That’s why we encourage all Hennepin Health members to visit the dentist regularly.
Your dental plan covers:

- Restorative work
Everyone one year and older should see a dentist twice a year. Even if you don’t have your natural teeth, you should still go to the dentist. People who wear dentures should get checked at least once a year.

Members (ages 1+) can earn a $25 Target or Walmart gift card for completing an annual dental visit by a DDMN network dentist. To get a gift card, make an appointment with an in-network dentist for an annual exam. After your dental visit, fill out this form. We will process your request and mail you a gift card.
Breast cancer is the second most common cancer in women in the U.S. — but catching it early can make all the difference. A simple X-ray called a mammogram can spot signs of cancer before you feel anything. If you're 40 or older, it's time to start getting checked every year. And if breast or ovarian cancer runs in your family, you may need to start even sooner.
Earn a $50 gift card
Hennepin Health members (ages 40-74) can earn a $50 Target or Walmart gift card for completing a mammogram. After your screening, submit a mammogram screening voucher: English Af Soomaali Español. We will process your request and mail you a gift card.

As a Hennepin Health member, you have gym benefits. This means you have discounted or free access to all YMCA of the North locations and free access to the Touchstone Fitness Center (part of Touchstone Mental Health) in Minneapolis.
All Hennepin Health members
Hennepin Health offers our members free access to the Touchstone Fitness Center, located at 2312 Snelling Ave., Minneapolis. Bring your insurance card to Touchstone to sign up. Call 612-314-1020 for more information.
MinnesotaCare and PMAP members
Hennepin Health offers a gym membership credit to MinnesotaCare and PMAP members who belong to the YMCA of the North. With this credit, YMCA members can save up to 40% off their monthly membership fee. When a member visits the YMCA six times a month, a $30 credit is applied to their account for that month.
MinnesotaCare and PMAP members who already belong to the YMCA can show their Hennepin Health member ID card at the YMCA check-in desk and ask to be enrolled in the Hennepin Health reimbursement program.
MinnesotaCare and PMAP members who want to join the YMCA can sign-up online or in person at any YMCA of the North location. Once you are a YMCA member, show your Hennepin Health member ID card at the YMCA check-in desk to enroll in the Hennepin Health reimbursement program.
SNBC members
Hennepin Health offers SNBC members a free gym membership at YMCA of the North.
SNBC members do not pay an enrollment or monthly membership fee. Hennepin Health pays their monthly membership costs after their first visit of each month.
If you are an SNBC member, simply show your Hennepin Health member ID card when you check in to any YMCA of the North location.
