When it comes to your health insurance, two important activities are renewing your eligibility and enrolling in a health plan. These are two separate activities. Read on to learn more about each activity and what actions you need to take.
Member eligibility renewal
Each year, you must renew your health insurance eligibility. Your renewal date is based on the anniversary of your original enrollment date. You can look up your renewal date at www.mnrenewallookup.com.
When it’s your time to renew, the Minnesota Department of Human Services (DHS) will mail you a notice and a packet with instructions. Look for an envelope with a blue circle marked “Important information enclosed.” Make sure to reply on time. You must act or you will lose your health insurance.
Learn more about the eligibility renewal process at www.hennepinhealth.org/members/renew-my-coverage.

Annual health plan selection
The annual health plan selection period begins in October and ends in early December. This is an opportunity to select a different health plan for 2026, if you wish to do so. DHS will mail you a notice in the coming weeks explaining the process. You can also visit mn.gov/dhs/health-plan-selection/ for details.
If you wish to keep Hennepin Health as your health plan for 2026, you do not need to take any action during the annual health plan selection period.

We all want to have a say in our medical care, but we may not always be able communicate with our providers due to illness, injury, or other circumstances. That’s why having an advance directive (also called a “health care directive”) is so important. It offers you peace of mind that your family and health providers will honor your preferences about life-sustaining treatments.
An advance directive is a written document you give to your health care provider in which you state your goals and wishes for current and future health care. This allows your voice to be heard when treatment decisions need to be made if you are unable to communicate them. It also enables you to name a person to make health care decisions for you if you are unable to do so.

If you have diabetes, getting your yearly health screenings is very important — especially your diabetic eye exam. This exam checks for damage to the retina, which can happen when blood sugar levels are too high. This condition is called diabetic retinopathy and can lead to vision loss or blindness if not caught early.
The good news? Regular eye exams and managing your blood sugar and blood pressure can help protect your vision. Hennepin Health members ages 18 and older can earn a $25 gift card for getting their annual diabetic eye exam.
