How do I find a provider or clinic?

Medical and mental health care providers

Hennepin Health has a wide provider network. Use Hennepin Health’s searchable online provider directory to find a provider or clinic. You can also look at the PMAP/MNCare provider directory or the SNBC provider directory.

Dental providers

To find a dentist who is accepting new patients, call our partner, Delta Dental, at 651-348-3233 (866-298-5549), Monday-Friday, 8 a.m. –  5 p.m. Learn more about your dental care at hennepinhealth.org/members/dental-care

What happens if I didn’t get my member ID or I if I lose my member ID?

Hennepin Health sends new members a member ID in the mail. Contact Member Services immediately at 612-596-1036, TTY 711, if:

  • You do not receive your member ID
  • Any of the information on your member ID is wrong
  • You lose your member ID
  • Your personal information changes (phone number, address, etc.)

 

What if I need help with housing, food, or other services?

The Hennepin Health Social Service Navigation team can help you:

  • Access basic needs like housing or food
  • Connect to other Hennepin County services like SNAP, WIC, or General Assistance
  • Understand your care if you are confused or overwhelmed by a diagnosis

Call the Hennepin Health Social Service Navigation team at 612-348-8400. Leave a message on their secure voice mail with your name, date of birth, what you need assistance with, and a good phone number where they can reach you. A case worker will get back to you and help you get connected to resources.
