Watch your mail in January 2025. We’ll be sending you a Hennepin Health member ID card and details about your member benefits, when and where to get care, and how to take advantage of the Hennepin Health rewards program.
In addition, many Hennepin Health members will receive a communication in January from the Minnesota Department of Human Services (DHS), inviting them to complete a survey called the Consumer Assessment of Healthcare Providers and Systems (CAHPS®). It helps measure your satisfaction with Hennepin Health, your community health plan. We encourage you to take the survey if you receive it.
As a Hennepin Health member, you are eligible for COVID-19 and flu vaccines and at-home COVID-19 tests at no cost to you.
You can get a flu shot at your health clinic, local pharmacy, and some retail locations. Check this list of pharmacies for a location near you or go to https://www.health.state.mn.us/people/ immunize/basics/vaxfinder.html.
It’s not too late to get vaccinated for COVID-19 and flu. You’ll help protect your health and the health of those around you.
You also are eligible for free at-home COVID-19 tests. Just ask your primary care provider to prescribe an at-home COVID-19 test and pick it up at your pharmacy.

Our team is here to help. It’s made up of caring people like Trellis Johnson. On any given day, Trellis might help a member schedule transportation to a medical appointment, find an in-network provider, or update their contact information.
For Trellis, helping a member is like helping a family member, friend, or neighbor. “I was raised in North Minneapolis,” she says, noting that it’s nearly impossible to take a walk or go out to lunch without running into someone she knows. “I’m passionate about the city and county. It’s very personal to me. We have members who struggle with homelessness, substance use disorder, terminal illnesses. I really like talking to members and assisting them.”
Trellis counts pharmacy-related calls as some of her most meaningful interactions. “When a member’s prescriptions are not going through, it can literally mean life and death,” she says. “We mark it as an urgent issue and work with other teams to resolve it quickly.”
Small things can sometimes make a big impact on your well-being. Hennepin Health members in need of essentials such as clothing, backpacks/bags, or hygiene products are welcome to visit our Member Service Center, 525 Portland Ave. S., 8th floor, in downtown Minneapolis. Hours are Monday - Friday, 8 a.m. - 4:30 p.m. A Member Services representative will connect you with our Social Services Navigation staff, who will assist you with items that best meet your needs.
While you’re visiting the Member Service Center you can also:
- Get help finding a doctor, clinic, or dentist or making an appointment
- Pick up or reload a bus/rail pass
- Get help with reward vouchers
- Ask questions about your health plan benefits
- Get help with health insurance billings
- Use a private room to make a phone call
- Use the computers to look up health info or bus/rail maps and fares
- Gather community or Hennepin County materials and get information about programs like WIC, SNAP, and other services
Dr. Krishnan Subrahmanian (also known as Dr. Krish) is chief medical officer for Hennepin Health. Throughout his career, Dr. Krish has seen the importance of health insurance. But he also understands how confusing health insurance can be. It can be especially complex for those who are eligible for Medical Assistance and MinnesotaCare.
Dr. Krish and his Hennepin Health colleagues recently developed a four-part podcast that helps you understand and navigate Medical Assistance and MinnesotaCare.
In the podcast, Dr. Krish answers basic questions like who is eligible for Medical Assistance and MinnesotaCare, what the benefits are, and how to sign up and switch plans. He talks to community members who’ve used the programs, and interviews experts like Dr. Nathan Chomilo, Minnesota Department of Human Services medical director for Medicaid and Medical Assistance.
“We want everyone to understand how Medical Assistance and MinnesotaCare help our neighbors reclaim health and benefit our community,” says Dr. Krish. “Finding, choosing, enrolling, and switching health insurance plans is easy and we can help.”

Dental care is just as important as other medical care and can positively impact your overall health. We strongly encourage our members to see the dentist regularly to ensure good dental health.
Your Hennepin Health dental coverage includes check-ups, cleanings, fillings, and restorative work. Everyone ages one year and older — even people who don’t have their natural teeth — should see a dentist twice a year. Denture wearers should be checked at least once a year.

Members (ages 1+) can earn a $25 gift card for completing an annual dental visit by a Delta Dental network dentist. To get a gift card, make an appointment with an in-network dentist for an annual exam. After your dental visit, print the dental rewards voucher and follow the instructions to return the voucher to Hennepin Health. We will process your request and mail you a gift card.
