Understanding the difference between eligibility renewals and annual health plan selection

When it comes to your health insurance, two important activities are renewing your eligibility and enrolling in a health plan. These are two separate activities. Read on to learn more about each activity and what actions you need to take.

Member eligibility renewal

Each year, you must renew your health insurance eligibility. Your renewal date is based on the anniversary of your original enrollment date. You can look up your renewal date at https://www.mnrenewallookup.com/.        

When it’s your time to renew, the Minnesota Department of Human Services (DHS) will mail you a notice and a packet with instructions. Look for an envelope with a blue circle marked “Important information enclosed.” Make sure to reply on time. You must act or you will lose your health insurance.

Annual health plan selection

The annual health plan selection period begins in October and ends in early December. This is an opportunity to select a different health plan for 2025, if you wish to do so. DHS will mail you a notice in the coming weeks explaining the process. You can also visit https://mn.gov/dhs/health-plan-selection/ for details.

If you wish to keep Hennepin Health as your health plan for 2025, you do not need to take any actions during the annual health plan selection period.

You can get care from a wide provider network

As a Hennepin Health member, you can get care from a wide range of providers, clinics, and hospitals. Our network includes Allina Health, Children’s Minnesota, Hennepin Healthcare, North Memorial, M Health Fairview, NorthPoint Health & Wellness Center, and many more.

Hennepin Health Dental Days offer convenient dental care

We want to make getting dental care easy for our members. During our Dental Days at the Hennepin Health Member Service Center in downtown Minneapolis, members can receive care from expert providers from Children’s Dental Service (serving adults and children). 

Members can make an appointment to get dental care at one of the Dental Days throughout 2024. See below for dates and times and visit https://www.hennepinhealth.org/members/dental-care for details.

To make an appointment to receive care at Hennepin Health Dental Days, speak with the Hennepin Health Member Services team in person at the Member Service Center (see address above) or call Member Services at 612-596-1036, TTY 711, Monday-Friday, 8 a.m.-4:30 p.m.

Join us at Hennepin Health Wellness Day

Looking for a convenient way to get a mammogram, dental exam, or flu and COVID-19 vaccines? Come to the Hennepin Health Wellness Day, Wednesday, October 16 from 1-4 p.m. at the Hennepin Health Member Service Center, 525 Portland Ave. S., 8th floor, in downtown Minneapolis. The first 50 participants who are Hennepin Health members will be eligible to receive a $15 Target or Walmart gift card. We look forward to serving you. Just walk in. No appointment needed.

Don’t wait for symptoms: get your mammogram starting at age 40

Breast cancer is the second most common cancer for women in the U.S., and early detection is key.

The most common test for breast cancer is a mammogram. A mammogram takes pictures of the inside of a person’s breast. Most persons assigned female at birth should have yearly mammograms starting at age 40. Those who have family members with breast or ovarian cancer may need to start breast cancer screening sooner.

Hennepin Health members can get mammograms at a wide range of providers, clinics, and hospitals, including Allina Health, Hennepin Healthcare, North Memorial, M Health Fairview, NorthPoint Health & Wellness Center, and many more.

Earn a $50 gift card

Do your children need immunizations? Mobile clinic comes to you

The Hennepin Healthcare Pediatric Mobile Clinic is a clinic on wheels that meets you where you are and provides children’s healthcare.

The mobile clinic serves Hennepin Health members, providing children’s health exams, immunizations and more. It is staffed by an outreach nurse, nurse practitioner, Spanish-speaking community health worker, and other medical professionals and offers Spanish-speaking interpreters.

To schedule the mobile clinic to visit your home, call 612-430-2251. Leave a voicemail with your name and phone number. Hennepin Healthcare staff will follow up with you on the details.
