Renew your health insurance eligibility when you receive a notice from the Minnesota Department of Human Services

Your health insurance eligibility must be renewed annually. Here’s what to expect and what you can do. 

  • Be prepared. Get ready to renew by visiting gov/dhs/renewmycoverage. Know when it is your time to renew by using the renewal lookup tool.
  • Update your contact information. Let us know if your address has changed.
    • If you have MinnesotaCare health insurance, contact Health Care Consumer Support at 651-297-3862 or 800-657-3672.
    • If you have Medical Assistance (PMAP or SNBC) health insurance, visit Hennepin County at hennepin.us/health-care-assistance or call Hennepin Health Member Services at 612-596-1036, TTY 711, Monday-Friday from 8 a.m. – 4:30 p.m.
  • Watch for information about how to keep your MinnesotaCare or Medical Assistance. When it’s your time to renew your health insurance, the Minnesota Department of Human Services (DHS) will mail you a notice and a packet with instructions. Look for the envelope with a blue circle marked “Important information enclosed.” Make sure to reply on time.
  • Upload your completed renewal documents online. Submit your health insurance renewal by emailing or uploading your application paperwork at Hennepin County InfoKeep. You can also upload scans or photos of your documents at Uploads / Minnesota Department of Human Services.

  • Update your address to ensure you get your health insurance renewal packet
  • Get a new health insurance renewal packet mailed to you if you need one

  • Find a MNsure navigator to help you complete your health insurance renewal paperwork

Be prepared to renew your health care coverage

Care coordination available for Hennepin Health members with Special Needs BasicCare (SNBC) coverage

All Hennepin Health members with Special Needs BasicCare (SNBC) coverage have access to care coordination. Our SNBC care coordinators are nurses, social workers and community health workers who help members connect and engage with primary care providers and the health care system.

Care coordinators help ensure that our Hennepin Health members living with disabilities have an equal ability to access the resources they need. They focus on:

  • Helping members take an active role in their care
  • Meeting with members to develop individualized support plans to improve overall member well-being
  • Linking members to health and community services

We place a major emphasis on meeting members where they are. This includes respecting and acknowledging their cultural context. We focus on health education, helping members choose their care, and involvement of family caregivers and community.

Are you due for a colorectal cancer or breast cancer screening? You can earn a $50 gift card.

About colorectal cancer

Colorectal cancer is the third most common cancer in the U.S. and the second leading cause of cancer death. It affects men and women of all racial and ethnic groups.

Colorectal cancer is a cancer in the colon and/or rectum. Most colon cancers start as a polyp that is an abnormal growth inside the colon or rectum. Some polyps may become cancerous if they are not removed. Colorectal cancer is treatable when it is discovered early. To find out more, visit the Healthwise Knowledgebase.

Get a colorectal cancer screening – earn a gift card

Your risk for colorectal cancer increases as you get older. Experts recommend starting screening at age 45 for people who are at average risk. To schedule a colonoscopy with Hennepin Healthcare or learn about options like home screening tests, call 612-873-6963.

About breast cancer

Breast cancer occurs when abnormal cells grow out of control in one or both breasts. These cancer cells can spread to nearby tissues and form a mass, called a tumor. Doctors don't know exactly what causes breast cancer. Some things are known to increase the chance that you will get it, such as your age and health history.

The most common symptom is a painless lump or thickening in the breast or underarm. But early breast cancer is often found on a mammogram before a lump can be felt. Breast cancer can be a curable disease if found and treated early. To find out more, visit the Healthwise Knowledgebase.

Get a breast cancer screening – earn a gift card

The most common test for breast cancer is a mammogram. A mammogram takes pictures of the inside of a person’s breast.

Most persons assigned female at birth should have yearly mammograms starting at age 40. Those who have family members with breast or ovarian cancer may need to start breast cancer screening sooner. To schedule a mammogram with Hennepin Healthcare, call 612-873-4500.

Get a ride to your health care appointments and related needs

Eligible Hennepin Health members can get a ride to and from health care services for:

  • Medical, dental and physical therapy appointments
  • Occupational therapy, acupuncture or chiropractic appointments
  • Mental health and chemical dependency appointments
  • Appointments with care coordinators (SNBC members or members who are assigned one)
  • Picking up your prescription at a pharmacy
  • Visiting the YMCA (for those covered by PMAP and SNBC)

You are eligible if you have Hennepin Health PMAP or SNBC coverage or have Hennepin Health MNCare coverage and are under age 19 or pregnant.

Ride options are a Metro Transit bus/light rail pass, taxi or specialized transportation. Pick up or reload a bus/light rail pass at the Hennepin Health Member Service Center, 525 Portland Avenue, Minneapolis, 8th floor. You can request a single,10-ride or monthly pass. The bus/light rail pass must be used specifically for the purposes listed above.

You also can call Hennepin Health Member Services at 612-596-1036 (TTY 711), Monday-Friday from 8 a.m. – 4:30 p.m. to request a bus/light rail pass be mailed to you. Call us at least three days before you need a ride using a taxi or specialized transportation.

Know your expanded 2024 dental benefits

For calendar year 2024, your Hennepin Health dental benefits have expanded. Here are the highlights:

Preventive Services

  • All members now have coverage for preventive services annually, such as nutritional and tobacco counseling for control of dental disease and counseling for control and prevention of adverse health effects associated with high-risk substance use

Prosthodontic Services

  • Prosthodontic arches allowed once every three years

Non-Pregnant Adult Member Services

Non-pregnant adult members have the following changes to their benefit coverage:

  • One detailed and extensive oral evaluation per calendar year and one comprehensive periodontal evaluation per calendar year; all oral exams should not exceed four per calendar year
  • Full mouth x-rays do not need to be taken in an outpatient hospital or freestanding Ambulatory Surgery Center (ASC)
  • Fluoride varnish once every six months
  • Oral hygiene instructions once per year
  • Sealants once per tooth per five years for permanent molars only
  • Single crowns and prefabricated crowns (stainless steel or resin)
  • Root canals for anterior teeth, premolars, and permanent molars once per lifetime
  • Retreatment of root canals
  • Occlusal adjustments
  • Limited and comprehensive orthodontic treatment

It's easy to get dental care at Hennepin Health Dental Days

We want to make getting dental care easy for our members. During our Dental Days at the Hennepin Health Member Service Center in downtown Minneapolis, members can receive care from expert providers from Children’s Dental Service (serving adults and children).
