Request for proposals: Community-based cardiovascular and diabetes health support services

Hennepin County is soliciting proposals for cardiovascular and diabetes health support services.

This request for proposals (RFP) will fund community organizations to improve heart health among African American/Black and/or Indigenous women in Hennepin County through increased social-physical activities, cultural activities, group and peer support, and/or lifestyle changes. Hennepin County anticipates awarding up to 10 contracts from this RFP, ranging from $10,000 to $50,000 each.

Proposal timeline

• Proposal due date: March 1, 2024 by 2 p.m.
• Pre-proposal conference: February 12, 2024 at 2 p.m.

How to submit a proposal

• RFP materials are posted in the Hennepin County Supplier Portal. Select Contracting Opportunities, and look for the RFP titled “Cardiovascular & diabetes health support services". 
• Providers will submit proposals via the Hennepin County Supplier Portal. Providers who do not already have a Supplier Portal account first need to register with the Supplier Portal in order to submit their application.
