MinnesotaCare members: It’s time to renew your eligibility

You should have received a packet in the mail recently from the Minnesota Department of Human Services (DHS) about how to renew your MinnesotaCare. Some members will automatically renew but will still receive a notice. Look for the envelope with a blue circle marked "Important information enclosed." Return the requested materials by January 31 to prevent losing your health care with us. You can keep your Hennepin Health plan.

We want to help you avoid losing your health insurance and creating possible disruptions in seeing your doctor or filling a prescription. If you didn’t receive your packet in the mail from DHS, contact us to:

  • Help you update your address.
  • Help you get a new packet.
  • Answer questions about your coverage.

This is different from the annual health plan selection

Your member eligibility renewal is different from the annual health plan selection (AHPS); they are two separate things. AHPS is an opportunity for you to change your health plan if you want. The AHPS period ended in early December.

New member ID cards coming in January

All Hennepin Health members will receive new member ID cards in January. The cards have a new phone number (833-210-5966) for Navitus, our pharmacy vendor.

The ID cards, along with a summary brochure of your member benefits, will be mailed in January. Destroy your old ID card once you get the new one.

If you don’t receive your new member ID card by January 30, please contact Member Services at 612-596-1036, TTY 711 for assistance.

Also coming in January

The annual member satisfaction survey, called the Consumer Assessment of Healthcare Providers and Systems (CAHPS®) survey, will be randomly mailed by DHS to members in early 2024. The CAHPS survey helps us measure your satisfaction with our plan. We encourage you to take the survey if you receive it.

Dental care is health care

Your member benefits include dental care. Keep your teeth healthy by practicing basic dental care:

  • Brush and floss regularly
  • Eat a healthy diet
  • Visit your dentist for regular checkups and cleanings

Developing good dental health habits is the best way to prevent tooth decay and gum disease. If you haven’t had a dental visit recently, make an appointment to keep your teeth healthy.

Health club membership credit for PMAP members

Starting January 1, 2024, Hennepin Health will offer a health club membership credit to PMAP members who belong to the YMCA of the North. When you visit the YMCA six times in the month, a $30 credit is applied to your account. With this credit, you can save up to 40% off your monthly membership fee.

The program is newly available to PMAP members who currently have a YMCA membership or who join the YMCA. You may qualify for other financial assistance offered by the YMCA.

How to get the credit

After January 1, PMAP members who already belong to the YMCA can show your Hennepin Health member ID card when you check in for your next YMCA visit and ask to be enrolled in the Hennepin Health reimbursement program.

PMAP members who want to join the YMCA can sign-up online or in person. Go to Locations | YMCA of the North (ymcanorth.org) to find the YMCA locations near you. Once you become a YMCA member, show your Hennepin Health member ID card at the YMCA check-in to become enrolled in the Hennepin Health reimbursement program.

Note for SNBC and MinnesotaCare members: Your YMCA health club benefit remains the same. This health club membership is expanding to PMAP members.

Enrollee Advisory Council update

The Enrollee Advisory Council (EAC), who are Hennepin Health members, met on November 16. Members discussed the following topics:

  • The substance use disorder (SUD) re-investment project
  • A member portal
  • Upcoming EAC election

Albert Garcia talked about the recent Quality Award given by the Minnesota Hospital Association to Hennepin Health and Hennepin Healthcare for our partnership project on SUD. This new program connects members who have SUD and an emergency department or inpatient hospitalization with a certified peer support specialist for additional help.

EAC members discussed their thoughts and suggestions for a possible member portal. Members were reminded that their term on the EAC ends in early 2024. Current EAC members and all Hennepin Health members are invited to apply for the 2024-2025 EAC term. Applications will be sent out in January.

The EAC gives feedback on overall health care issues, new programs and changes in procedures. Group feedback reflects the priorities and interests of members. If you have questions about the EAC, call us at 612-596-1036, TTY 711.

Telehealth is one of your health care options

Telehealth is a covered benefit for all members. It can be a good option to treat many common conditions from the comfort of your home.

When to use urgent care

Urgent care is for a condition that needs prompt treatment but is not life-threatening. You have an open network for urgent care.

North Market blood pressure chair

North Market BP chair

We are pleased to fund a blood pressure chair that was placed at North Market to support community members. The chair measures a person’s blood pressure to give them a current reading. It also contains information to understand what their blood pressure number means and what they can do to lower their blood pressure.

North Market is a grocery store in north Minneapolis that works in partnership with Pillsbury United Communities. The store has community health workers who answer health questions and give health education and resources.

The blood pressure chair can be life saving for a community that has fewer places to go for groceries and health information. North Market is located at 4414 Humbolt Avenue N.

Be prepared for winter weather

Minnesota winters can be a great time to get out and explore. But winter also presents some hazards to prepare for including extreme cold, snow and ice.

  • Find Hennepin County severe cold warming options at us/STAYWARM.
  • Call the Hennepin Shelter Hotline (612-204-8200) with questions about warming options. The hotline is always available, not just on dangerously cold days.

Advance health care directives

You can inform others of your health care wishes with an advance health care directive. A health care directive is a written document that informs others of your wishes about your health care. It allows you to name a person (agent) to decide for you if you are unable to or if you want someone else to decide for you. You must be at least 18 years old to make a health care directive.

A health care directive is important if your attending physician determines you can’t communicate your health care choices because of physical or mental incapacity. It is also important if you wish to have someone else make your health care decisions. In some circumstances, your directive may state that you want someone other than an attending physician to decide when you cannot make your own decisions.

You don’t need to have a health care directive. But writing one helps to make sure your wishes are followed. You will still receive medical treatment if you don’t have a written directive. Health care providers will listen to what people close to you say about your treatment preferences. But the best way to be sure your wishes are followed is to have an advance health care directive.
