Medical Fragility Feedback Needed

 

Prioritization based on Medical Fragility rolled out in the Singles system on August 1st, 2023.

Now we want your feedback on the medical fragility questions. 

Specifically, we want to know any assessments where you believe the answers the client gave to the medical fragility questions did not reflect their actual level of fragility.

It has been brought to our attention that there may be a difference between how the medical fragility questions are answered by the person experiencing homelessness and what the CES assessor, trained case manager, advocate, or social worker may observe and experience with the person completing the questions. 

We know that sometimes case conferencing is used in these situations to make sure that the autonomy of people experiencing homelessness is respected, while attempting to ensure that their access to homeless dedicated housing is not negatively impacted. 

Let us know here if you believe there is a discrepancy between how the medical fragility questions were answered by the client and their actual circumstances, thus negatively impacting their prioritization for a housing referral.

After we collect your responses we will bring them to the Lived Experience Advisory Group for further discussion.

Heading  Home Hennepin Annual Meeting

 

The Heading Home Annual Meeting is an opportunity for those who work to prevent and end homelessness in Hennepin County to come together, network, and learn from one another. 

This year you’ll hear from:

  • Minneapolis Mayor Jacob Frey - Co-Chair of the Heading Home Hennepin Executive Committee.
  • David Hewitt - Director of Housing Stability at Hennepin County.
  • Rico Morales - Co-Chair of the Lived Experience Advisory Group (LEAG).
  • Cathy ten Broeke - Assistant Commissioner and Executive Director of the Minnesota Interagency Council on Homelessness. 

The event will also feature three breakout sessions, with each centering on making homelessness rare, brief, and nonrecurring.
