Remember to update Current Living Situation in the assessment anytime it changes.
Housing Providers should especially keep this top of mind when:
- Completing intake with the client.
- During the housing search.
Assessors should keep this top of mind when:
- Doing monthly check-ins with clients they assessed.
- Anytime they learn the client's living situation changes.
As of August 1st, the Medical Fragility questions in the assessment impact prioritization for housing referrals.
Prioritization is now:
- Medical Fragility
- Chronic Homelessness
- # of HUD Months Homeless
Clients will not get a referral unless the Medical Fragility questions are answered completely.
