New HMIS Workflow for Housing Providers

 

There are new HMIS Workflows for Hennepin CES that impact Housing Providers in these scenarios:

In short, there are now fewer steps!

This is because you can now update Coordinated Entry Event on the final screen you see when removing a client from the priority list.

Fast Track Referrals are Back

 

Fast Track Referrals are Back!

You can now identify households in HMIS that are eligible for Fast Track referrals.

What is Fast Track?

Fast Track provides housing search and one-time financial assistance to families and single adults whose primary need is for deposit and move-in costs, and/or who need help with housing search and advocacy with owners and property managers.

Services Provided through Fast Track

  • Housing search assistance
  • Application help
  • Advocacy and mediation
  • One-time financial assistance needed to secure housing

Financial Assistance

This typically includes help with application fees, deposits, and possibly first/last month’s rent. People are expected to use EA or EGA to cover these costs first though if possible.

Other Helpful Things to Know

Clients are discharged upon move-in and need to maintain their housing without additional services or rental assistance from the Fast Track program. 

This means they need income or a subsidy that makes it possible to pay their rent ongoing (e.g. Earned Income, Housing Choice Voucher, Disability-Related Income, etc).

Income Eligibility for Families

  • Annual gross income at or below 200% of the Federal Poverty Level
  • Have either a Section 8 (or equivalent) subsidy, or...
  • Monthly income of $1,100 for a family of two (adding $100 for each additional household member).

Income Eligibility for Single Adults 

  • Annual gross income at or below 200% of the Federal Poverty Level
  • Have either a Section 8 (or equivalent) subsidy or,...
  • Monthly income of $1,500 - $2,000.
