Medical Fragility Questions Are Coming to the Singles Assessment on January 25th

Three Key Points About these New Questions:

1 - These new questions will focus on vulnerabilities related to the client's health. 

2 - In the near future, answers to the medical fragility questions will impact how clients are prioritized for housing referrals.

3 - In the meantime, make sure to update existing client assessments so the medical fragility questions are answered!

Here is how the questions will look in HMIS:

What is Medical Fragility?

Chronic health conditions that affect a person’s day to day quality of life.  

Examples of some chronic health conditions are high blood pressure, diabetes, congestive heart failure, and more.     

 

Why does Hennepin CES want to include Medical Fragility in the Singles Assessment?

Health and homelessness are intrinsically intertwined. Having a home can change the trajectory of a person’s life and health. 

People with chronic health conditions can improve their health and quality of life with stable housing. Without stable housing, people with chronic health conditions tend to die earlier. 

In order to ensure those experiencing the most severe health issues receive housing sooner, prioritizing them for housing is key.

The idea for this actually came from this group in January 2020 when a planner (Tracy Schumacher) attended a meeting.

One of the attendees asked why someone with minimal health needs or condition would get housed faster than someone with a chronic health condition. It was a very good question and one that we continued to discuss ever since. 

 

How was the series created?

A team of health professionals partnered with our housing/homeless team and created these questions.

Throughout the process, people with lived experience reviewed the series of questions and provided valuable feedback.

The health professionals that participated are Susannah King of Healthcare for the Homeless, Dr. Danielle Robertshaw of Hennepin County Public Health Clinical Services, and Dr. Katherine Vickery, researcher at Hennepin HealthCare
