When clients get referred to a housing provider, these 3 documents are commonly needed:
- ID
- Birth Certificate
- Social Security Card
Not having these documents dramatically slows down the path to housing (and other life opportunities like employment).
When you meet with a client, ask if they have these documents.
If YES (Client has an ID, Birth Certificate or Social Security Card):
- Take a picture and upload it to their Client Profile in HMIS in the "File Attachments" section.
- Don't have HMIS access? Email the photos to ces.hennepin@hennepin.us
If NO (Client does not have the documents):
Client Documents are Stored in the Client Profile in HMIS
- Click on the Client Profile tab.
- Scroll Down to File Attachments.
- Click on the edit pencil next to the document you want to view.
- Click on Download.
