Vital Documents Clients Need

 

When clients get referred to a housing provider, these 3 documents are commonly needed:

  • ID
  • Birth Certificate
  • Social Security Card

Not having these documents dramatically slows down the path to housing (and other life opportunities like employment).

When you meet with a client, ask if they have these documents. 

If YES (Client has an ID, Birth Certificate or Social Security Card):

  • Take a picture and upload it to their Client Profile in HMIS in the "File Attachments" section.
  • Don't have HMIS access? Email the photos to ces.hennepin@hennepin.us

If NO (Client does not have the documents):

Client Documents are Stored in the Client Profile in HMIS

  • Click on the Client Profile tab.
  • Scroll Down to File Attachments.
  • Click on the edit pencil next to the document you want to view.
  • Click on Download.
