Hello MRCs,
This email is regarding the vaccine attestation requirement for all Hennepin County staff and volunteers that was passed in September of 2021. Read the full COVID vaccine attestation requirement at the bottom of this email.
If you are fully vaccinated and have already shown proof of vaccination to Hennepin County, you do not need to take any further steps and do not need to read this email any further.
Starting June 1, we will resume sharing in-person volunteer opportunities. All Hennepin County volunteers are required to be fully vaccinated against COVID-19 and submit proof in order to continue volunteering with the county.
Here is the process to submit proof of vaccination:
Step 1
Fill out the COVID-19 vaccine attestation form. Here is information you’ll need to answer the questions:
Are you submitting for yourself or another staff member? Myself I am a Volunteer
County department you work in: Environment and Energy
Supervisor name you report to: Kaitlin Steinberg
My work location is: Remote
You will receive a confirmation email when your vaccine attestation is received.
Step 2
Send acceptable proof of vaccination to HR.COVID@hennepin.us.
Acceptable proof of vaccination means a copy, photo, or scan of one of the following:
- COVID-19 vaccination record card from the CDC
- A vaccination record from your medical provider (such as MyChart) or health insurance portal
- The Docket app
- Other vaccination record, such as from Minnesota Immunization Information Connection
- All documentation needs to include the name, DOB, and date(s) for the vaccine and type of vaccine received

COVID Vaccine Attestation Requirement
Starting September 1, 2021, all volunteers must be fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a volunteer opportunity offer is made, the volunteers must show proof of full vaccination prior to their first volunteer shift. A COVID-19 vaccination card from the Center for Disease Control and Prevention (CDC), a vaccination record from your doctor’s office or a personal digital vaccination record constitutes sufficient “proof.” The volunteer must also attest they are vaccinated by completing a form prior to their first volunteer shift. Reasonable accommodations to this vaccination requirement will be considered upon request by a volunteer for medical or religious reasons as required by applicable law.
