Q. For how long should I attempt to make contact with a referral?

A. 5 Business Days

 

Q. What should I do if I can't make contact with the referral?

A. Remove them from the priority list.

 

Q. What should I do if I completely lose contact with the client after working with them for a while?

A. Remove them from the priority list.

 

Q. If the client was not able to find housing, should I change the referral to "unsuccessful" in Coordinated Entry Event?

A. No. The referral itself was successful since you started working with the client.

What is the Assessor's Role When a Client Gets Referred?

Assessors Role When a Client Gets Referred

The Housing Provider takes the lead role in contacting the client.

 

The Assessor supports the housing provider.

 

It is ultimately the housing provider's responsibility to make contact with the client. 

However, assessors often play a crucial role in helping the housing provider make contact with the client, and providing support until the client is housed.

When a client gets referred, the assessor and case manager are copied on the email the housing provider received.

Assessors, do the following:

  • Introduce yourself to the housing provider.
  • Help locate the client.
  • Stay engaged until the client finds housing.

  • This phone is answered Wednesday-Saturday, 7 a.m. to 4 p.m.
  • If the phone is not answered, leave a message and someone will return the call.
  • The caller will first answer questions to make sure they are eligible for coordinated entry.

If eligible, an assessment will get scheduled.

Assessments are scheduled on week days during regular business hours.
