This new online form will make it faster and easier for you to request a referral. 

What happens to the old PDF Referral Request Form?

We will accept the PDF referral form through March 8, 2022. After that, housing providers are required to use the online form.

Do you struggle to follow all of the correct steps in HMIS? If so, we created some new, intuitive guides to help! Check them out here.

 

Choose a Scenario and Get an Intuitive Step-by-Step Guide

The new HMIS guides are based on common scenarios for housing providers. This way, you will know exactly what to do for each scenario. 

Here are the scenarios:

  • I just got a referral.
  • The referral is not eligible for our housing program.
  • I was not able to make any contact with the client.
  • I made contact with the client, but we are NOT moving forward.
  • I made contact with the client and intake is completed. We ARE moving forward.
  • The client moved into housing through the CES system.
  • The client exited our program without housing because they are no longer eligible for our program, or they no longer want to work with us.
  • The client exited our program without housing because we lost contact with them (jail, treatment, disappered, etc)

Katie DeSantis

For the last 4 years Katie DeSantis has served as Referral Coordinator in the Hennepin CES system. She has provided dedicated, skilled and friendly service to housing providers, assessors, and clients. 

We will miss Katie as she moves to the next stop on her her journey.

She is moving to Portland, Oregon to serve as Housing development coordinator with Portland Community Reinvestment Initiatives.

To get a housing assessment, call Matrix Housing Services at 612-283-2567. 

  • This phone is answered Wednesday-Saturday, 7 a.m. to 4 p.m.
  • If the phone is not answered, leave a message and someone will return the call.
  • The caller will first answer questions to make sure they are eligible for coordinated entry.

If eligible, an assessment will get scheduled.

Assessments are scheduled on week days during regular business hours.
