Housing Providers: Acknowledge Referrals in HMIS Within 2 Business Days

When housing providers get a referral, it is essential that they acknowledge it in HMIS within 2 business days.

Why is it so important to acknowledge referrals in HMIS within 2 business days?
- The client is a person with the highest vulnerability and need in the system.
- The longer you wait to acknowledge the referral, the longer they remain without a place to live.
This is especially the case if you decline the referral. The client will not get any other referrals while the system waits for the housing provider to acknowledge the it in HMIS.

If you decline the referral, or if the client chooses to not work with your program, state the reason why in the "Referral Notes" section of Coordinated Entry Event.

Doing an Interim Update?
Make sure "Section 1: Assessor Information" is filled out.

If you are doing an interim update on a client's assessment, double-check that "Section 1: Assessor Information" is filled out.
If it is not completed, it generates an error that prevents the client from appearing on the priority list.
Always double check Section 1: Assessor Information!

Homeless to Housing Program Eligibility
- 18+ years of age.
- No children in their care.
- Not working with a case manager.
- In a homeless shelter, sleeping outside or in a vehicle, camping or fleeing or attempting to flee domestic violence.

About the Homeless to Housing Program
This newly created program serves Hennepin County residents who are experiencing homelessness by providing housing case management assistance.
Homeless to Housing helps connect clients to resources, get vital documents like state IDs, birth certificates, and Social Security cards, and overcome barriers.
Referrals are now accepted!
Please make people aware of this program and urgently send in referral forms. This will help us quickly connect people with case managers.
