Homeless to Housing Program

Homeless to Housing Program Eligibility

  • 18+ years of age.
  • No children in their care.
  • Not working with a case manager.
  • In a homeless shelter, sleeping outside or in a vehicle, camping or fleeing or attempting to flee domestic violence.

About the Homeless to Housing Program

This newly created program serves Hennepin County residents who are experiencing homelessness by providing housing case management assistance.

Homeless to Housing helps connect clients to resources, get vital documents like state IDs, birth certificates, and Social Security cards, and overcome barriers.

Referrals are now accepted!

Please make people aware of this program and urgently send in referral forms. This will help us quickly connect people with case managers.

Assessors, please update assessments to help us determine if your clients are eligible for and interested in this permanent supportive housing.

 

What should you update in the assessment?

In the "CES Household Disability Information" section:

Select if an individual has a “Drug Use Disorder” or “Both Alcohol and Drug Use Disorder”

Referral Timeline Expectations

Referral Timeline Expectations

When I request a referral, how quickly can I expect a response?

Families System: 2 business days on average.

Singles System: 5 business days on average. 

If it will take more than 10 days, the CES team will let you know. 

Housing Provider Expectations Once a Referral is Received

  • Within 2 business days, acknowledge the referral in HMIS.
  • Within 1 business day after acknowledging the referral, contact the client by phone, text and email.
  • If after 24 hours there is no response from the client, contact alternative contacts, shelter advocates, outreach workers, and the assessors.
  • Continue daily contact attempts until an intake meeting is scheduled.
  • When you first make contact with the client, ask the best way to contact them moving forward (phone, email, text, alternative contacts, etc). 
  • If after 5 business days you don't make any contact with the client (e.g. You have no idea where they are, and neither does anyone else), remove them from the priority list.
