Hennepin County is soliciting proposals for homeless prevention, rapid rehousing and assessment services for the housing stability area of the Human Services and Public Health Department.
This includes activities which will rapidly rehouse homeless households, assist imminently homeless households with retaining their permanent housing, complete coordinated entry assessments for families and provide combined shelter diversion and coordinated entry assessments for youth.
The "Super RFP" anticipates awarding over $9 million in funding for the 2021-23 biennium and includes FHPAP, CoC, ESG, and County funds. The County will fund multiple proposals for rapid rehousing, multiple proposals or homelessness prevention, only proposal for family assessment and may fund at least one proposal for youth assessment and diversion.

How to submit a proposal
- RFP materials are posted in the Hennepin County supplier portal. Look for the RFP titled "Homeless prevention, rapid rehousing and diversion"
- Providers will submit proposals via the Hennepin County Supplier Portal. Providers who do not already have a Supplier Portal account first need to access the supplier portal information page to register.
