Coordinated Entry System 101 Training

 

What: CES 101 is designed to offer an overview of Hennepin County's Coordinated Entry System. This training will focus on the basics of what Coordinated Entry is, is not, access points, who it is intended to serve and what resources are available specifically in the Hennepin County system.

The intended audience for this training are new staff members, advocates or community agencies not familiar with the Coordinated Entry System.

This training will be offered once per quarter as a resource to assist training new staff in talking points, myths and questions.

Important reminder for Assessors: Make sure the CES ROI is singed and that it is reflected in HMIS

Anyone assessed for coordinated entry needs to sign the Hennepin Coordinated Entry System Release of Information (ROI). This release allows the sharing of data with all providers in the Hennepin Continuum of Care that serve as the homeless response system.

The date this form was signed must also be completed in HMIS. A screenshot of that field is below. Failure to fill in this date in tells the coordinated entry system that the client did not consent to having their information released and would therefore lead to them being removed from the priority list.

Important reminder for Housing Providers: Enter Housing move-in date and create an exit to the priority list. 

Housing Move-in Date

Housing Move-In Date is the date a household moves into housing. It is vital to record this date to track the length of time it takes for a household to move from the assessment to being housed. 

If there is no Housing Move-in Date then the client should be canceled/declined. All referrals should should have an outcome of Cancel/Decline or a Housing Move In Date.

Exit to the Priority List

When housing someone, please make sure to exit them from the priority list. This entails creating an Exit to their corresponding Entry onto the CES Hennepin Priority list.

Single adult and youth housing provider vacancies reported

Each month at the Coordinated Entry leadership committee members review the data of the previous month. Below is a snapshot of the vacancies reported by single adult youth housing providers as of September 1st, 2019. These vacancies are broken up by housing type. For Single adults the categories are Rapid Rehousing (RRH), Housing Support, and Permanent Supportive Housing (PSH). For youth, the housing types are Rapid Rehousing (RRH), Transitional Housing (TH), and Permanent Supportive Housing (PSH).

Single Adult and Youth Priority List

The total number of clients on the priority list is the number of individuals that have been assessed for Coordinated Entry and eligible for a referral. All these individuals have accessed services (i.e. drop-in, shelter, outreach, etc.) within the last 90 days. 

The pending referrals number is supposed to reflect the number of individuals that have a referral to a housing provider, but have not yet entered the program, however, we know this number is inflated due to some bad data. If a referral is not "exited" from the priority list after being accepted to a housing provider they will end up in this category. This is a good example of how it is hard to get accurate data if all steps aren't followed in your HMIS workflow.

The inflow (159) shows how many entered the priority list in the last month and the outflow (70) shows how many left the priority list in the last month. In this period the total number on the priority list increased by 89 individuals.
