If you provide
services to African American families receiving Minnesota Family Investment
Program (MFIP) assistance, please tell your clients about a new employment
services initiative that addresses racial disparities for families served by
MFIP. On Point’s career coaching helps participants focus on strengths to
create opportunities that will help them move beyond poverty and achieve
economic independence. On Point is located at North Point Health and Wellness
Center, where participants can access health care and human services support in

Providers,
let your clients know that Target is hosting a hiring event from 9:00 am to
noon, Wednesday, September 30th at the Minneapolis Central Library, located at
300 Nicollet Mall. The event will take place in the Robins, Kaplan, Miller and Ciresi Community Meeting
Room, 2nd floor (above Dunn Brothers) of the library. Positions are available at the St. Louis
Park and Ridgedale stores.
The Central/Northeast human service center is expected to open at the Health Services Building in downtown Minneapolis in November, making this location the fifth human service center to open in Hennepin County since 2012. A human service center located at Lake and Hiawatha in South Minneapolis is scheduled to open in early 2017.
All human service centers provide a full range of services and host specialty services delivered by onsite, nonprofit partners to help clients get services faster. Clients choose the center that works best for their location and schedule.

Northwest changes opening hours
The Northwest Suburban Human Service Center service hours will change from 7:30 to 8 a.m. beginning Monday, October 19. Closing time will remain 4:30 p.m.
Learn how seniors and persons with disabilities can get help to live in their communities as independently as possible. focus on accessing home- and community-based services and potential funding to remain at home longer. Sessions are intended for parents, providers, legal representatives and persons with disabilities.

The Minnesota Department of Health has sent a notice to Medical Assistance enrollees informing them of their choice of health plans for 2016. Fewer plans will be available for single adults, families and children who receive Medical Assistance. (This change does not affect seniors and disabled on Minnesota Senior Care Plus, Minnesota Senior Health Options, or Special Needs Basic Care coverage. See note below for more information about these plans.)
The reduction in plans means that a large number of clients will need to select a new plan. Plans that are no longer offered include UCare and HealthPartners. The three plans that will be offered in 2016 are:

Hennepin Health/Metropolitan Health Plan (expanding to include families)
- Medica
Clients who want to change plans can complete their selection on the forms they received in the mail and return them to the Department of Human Services: Minnesota Health Care Programs, PO Box 64984, St Paul, MN 55164-0984.
Clients who want information about the plans, such as a list of providers, can either check the appropriate box on the DHS form and mail it in or call DHS at 651-431-2670 or 1-800-657-3739. DHS will send the client the information.
Clients currently enrolled in UCare or HealthPartners (two plans not offered in 2016) who do not respond to the DHS letter will automatically be enrolled in Medica for 2016.
Clients currently enrolled in Medica or Hennepin Health who do not respond to the DHS letter will remain in that health plan for 2016.
Clients who have questions about their current health plan or medical services may call the Managed Health Care Advocate at 612-596-8860.
Note: For senior and disabled persons covered under Minnesota SeniorCare Plus (MSC+), Minnesota Senior Health Options (MSHO) or Special Needs BasicCare (SNBC), the plans available in 2016 will be the same as in 2015. During annual health plan selection, these enrollees have the option to change plans if they would like to. Contracting
information sessions are offered to support your ability to meet contracting
requirements. Pre-registration is required. To pre-register, email county staff
with the names of all attendees, their email addresses and a contact telephone

contract monitoring and the vendor compliance audit
Learn
the elements of monitoring (administrative, financial, and programmatic) and
what to expect when selected for a financial audit. This session is intended
for providers who currently have a contract with the Human Services and Public
Health Department.
