Important revalidation process reminders for high-risk service providers

We have learned in recent weeks that some important steps in the revalidation process may be misunderstood by providers and are being missed. Please review the following for clarifying information.

Report change of ownership

It is important to let DHS know when there is a change of ownership at a provider organization. Change in ownership may result in payment reversals for terminated providers. Refer to Provider Entity Sale or Transfer Addendum (DHS-5550) for detailed instructions.

Disclose business operation involvement

Up-to-date information on managing employees, board members, and other individuals involved in daily business operations must be kept on record. For specifics, read What role types am I required to disclose and what are the definitions for owner, managing employee, and other roles? in the online FAQ. The definitions section of Disclosure of Ownership and Control Interest of an Entity (DHS-5259) may also be helpful.

Questions about revalidation appeals termination dates?

DHS has received many questions from providers currently appealing a revalidation termination made for their enrollment records with high-risk services as part of Minnesota Revalidate 2026. Please refer to a July 14, 2026, provider news update for clarifying information.


Enrollment moratorium on metro area NEMT providers extended

The enrollment moratorium on Non-Emergency Medical Transportation providers who are based in the seven-county metro area has been extended. Get more details in the provider news message posted on July 23, 2026.


Guidance and resources on obstetrics services coding

Currently, providers can bill obstetric services globally or by component. However, on Jan. 1, 2027, the American Medical Association is eliminating the global CPT codes. Global codes bundle all prenatal visits, delivery, and postpartum care into a single claim. Under the new system, each phase of care will be billed separately as services are provided. DHS will follow updated billing guidance from the American College of Obstetricians & Gynecologists, posted in a July 2 provider news message on our site.
