The provider revalidation FAQ page has been updated with new content about disenrollment. If you do receive a disenrollment notice, make sure to read it thoroughly and act on it as soon as possible. If you have questions about the appeals process, please use the provider appeals email inbox.
We have learned that some providers have received phone calls from unauthorized parties requesting details about enrollment records and private health information, under the guise of completing the revalidation process.
Department staff will never request patient or recipient information. As part of revalidation, DHS representatives may request staff information — limited to individuals who meet disclosure requirements — but never over the phone. DHS may also ask about required professional or licensed affiliated staff, including roles, responsibilities, and Medicaid affiliations.

The department has added an extra Waiver and AC Resources and MN–ITS Training session for HCBS waiver providers completing revalidation requirements: June 2–3, 9 a.m. to 1 p.m.

Due to the high volume of inquiries about revalidation, callers to the Provider Resource Center will likely experience longer than usual wait times. Consider using the PRC's online contact form to submit your question.
On May 30, MN–ITS will move to LoginMN, the new secure platform for all state services.
To maintain MN–ITS access, providers must validate contact info before the switch.
If you have not validated yet, when you login to MN–ITS you will see a pop-up with instructions. No pop-up means you're all set. More details will be posted on Provider News in coming days.
In partnership with the University of Colorado Anschutz Medical School, the department has launched a survey to understand how providers experienced the period of heightened federal immigration enforcement activity between December 2025 and February 2026. The survey closes June 26. Responses are anonymous.
