The Maltreatment of Minors Mandated Reporting form was updated after some of the programs previously licensed by the Minnesota Department of Human Services (DHS) transitioned to the Department of Children, Youth, and Families (DCYF). There are updated forms and new links for programs that continue to be licensed or certified by DHS.
