Disability Hub MN grant opportunity
DHS is accepting applications from interested parties for a grant opportunity to run and staff the Disability Hub MN contact center with support from DHS. The selected responder will:
- Be committed to equity, inclusion and person-centered, unbiased support for people with disabilities in Minnesota.
- Help people access accurate information, useful resources and support that reflects their needs and choices.
- Deliver information, resources and support without bias or conflicts of interest.

Application: To apply, refer to DHS – Open grants, RFPs and RFIs – DHS is seeking for grantee to host the Disability Hub.

Number of grantees: One.

Amount: Up to $550,000 for transition planning and hiring during year one (Dec. 1, 2025, to June 30, 2026) and up to $3,135,633 for full implementation during year two (July 1, 2026, to June 30, 2027).

Area Plan updates
Saving Claire program Falls Prevention Awareness Month calendar
We added a fillable calendar document titled “Saving Claire Fillable Calendar" for Falls Prevention Awareness Month (September 2025) to the MBA:AAA Teams channel, Healthy Aging Quarterly Meeting folder. AAAs should use the calendar to record Saving Claire events or other falls prevention activities happening in their region.
List falls prevention awareness events planned in your planning and service area prior to the next Healthy Aging Meeting on August 14,2025 so we can help spread the word through social media.
Saving Claire continues to grow nationwide, with new partnerships launching in Maryland, California, Minnesota, Missouri, and beyond—including major rollouts at Tampa Bay YMCA and the Chautauqua Institute. A featured presentation at the National Council on Aging's Age+Action conference and upcoming recognition at Gerontological Society of America (GSA) 2025 highlight the project's national impact.

Office of HIV/AIDS Network Coordination survey invitation
As part of a new campaign focused on HIV prevention efforts for older adults, the Office of HIV/AIDS Network Coordination (HANC) is inviting individuals 50 years of age and older to respond to a few questions related to sexual health, sexual wellness, intimacy, and HIV prevention efforts for individuals 50 and older. Completing the survey should take approximately 5-10 minutes.
