245G-licensed providers must obtain 245G.20 designation to be eligible to certify an ASAM level of care


The Department of Human Services reminds 245G-licensed providers that they must have the 245G.20 designation to be eligible to certify an ASAM level of care and participate in the 1115 SUD Demonstration. The 245G.20 designates that the provider is “Specializing in the Treatment of Persons with a Substance Use Disorder and Mental Health Disorders.”

To obtain the 245G.20 designation, you must work with your assigned licensor or email dhs.mhcdlicensing@state.mn.us with “245G.20 add-on” in the subject line to be assigned one.

Please note, you are not required to meet the standards listed under Chapter 254B.05 Subd. 5(c)(5)(ii) for co-occurring enhanced services.

The following substance use disorder providers are required to certify their ASAM level(s) of care by Jan. 1, 2025, or become ineligible for payment for services provided under Minnesota Health Care Programs (MHCP):

  • Minnesota outpatient treatment programs licensed under 245G by the Department of Human Services (DHS) and enrolled in MHCP
  • Minnesota residential programs licensed according to 245G.21 and that are licensed as a hospital under sections 144.50 to 144.581 that provide ASAM levels of care other than ASAM 3.7 medically monitored inpatient

Tribally licensed providers may elect to participate.

This is in accordance with Minnesota Statute 256B.0759 Subd. 2.

Certification Assistance
