The Department of Human Services has created a 245G Comprehensive Assessment Reference Guide as a tool for providers.

The guide is intended to remove more prescriptive practices and allow professionals to utilize their clinical expertise when providing substance use disorder services. The format is not required.

The 245G Comprehensive Assessment Reference Guide (PDF) is available for providers to access on the Direct Access Website located under Resources.
