QRTP Family and Permanency Team Meeting is available as a claimable activity for MH-TCM and CW-TCM, but the following conditions must be met when claiming for this activity. It is the local agency’s responsibility to ensure claims billed meet the Targeted Case Management (TCM) Medical Assistance (MA) reimbursement standards. A contact recorded under a claimable activity that does not meet these standards should be excluded from healthcare claiming. Please reference the Exclusions document on the SSIS Fiscal Documentation page for instructions.
Child Welfare Targeted Case Management (CW-TCM) T2023 U3
Claiming is allowed for CW-TCM when the QRTP Family and Permanency Team Meeting occurs prior to child being placed in the QRTP facility.
If the child is already in a QRTP at the time of the QRTP Family and Permanency Team Meeting, claiming is not allowed as QRTPs are MA-funded facilities. The only exception to this rule is that claiming can occur in the 30-day period prior to discharge. This exception is limited to two instances per calendar year.
The child or another individual identified in the service plan needs to be present at the QRTP Family and Permanency Team Meeting to claim CW-TCM. It is best practice to see the child every month. This individual can also be a collateral identified in the service plan (ex: foster parent, therapist, others relevant to goals of child as documented in the service plan).
Mental Health Targeted Case Management (MH-TCM) T2023 HE HA or T2023 HE
The eligible child, their parent, or the child’s legal representative must be present at the QRTP Family and Permanency Team Meeting to claim for MH-TCM. It is best practice to see the child every month.
Claiming MH-TCM is allowed for up to 180 days when the client resides in a MA-funded facility. This can be billed while the client is placed in a QRTP until the TCM benefit is exhausted. The first date of service will start the 180-day time limit for claiming. MMIS enforces this rule.

This webinar will be recorded and posted, along with the PowerPoint and additional reference materials, to the Minnesota Child Welfare Training Academy website seven to ten business days following the event.
We are looking to improve communications and strengthen relationships with end users, mentors, and county and tribal administrators. We are also mindful of making sure that every meeting has value for informing stakeholders and/or soliciting stakeholder input. We will be sending out a very brief survey in April regarding the utility of various formats and content of communication.
