This weekly digest is a new method to communicate updates, initiatives and policy changes from the Aging and Adult Services Division (AASD) and Disability Services Division (DSD) in a simplified way. Summary: Effective April 10, 2023, the Mini-Cog field on the LTC screening document will be a required field, and the mental status eval field will no longer be required. This change is necessary to begin the transition to the Mini-Cog in the MnCHOICES revision. The Mini-Cog is a screening tool to detect the possible presence of dementia. Certified assessors will use Mini-Cog for MnCHOICES assessments completed in the MnCHOICES revision for people age 30 and older.

Summary: The Minnesota Board on Aging requests public comment about the proposed State Plan on Aging for FFY 2024 to 2027. The board must submit a new State Plan on Aging to the federal Administration on Community Living (ACL) by July 1, 2023. The plan documents achievements and planned activities related to the state’s LTSS planning efforts and assures ACL the state will follow the provisions of the Older Americans Act. It also provides a framework for state activities related to programs, activities and services for older adults.

Summary: To prepare for the forthcoming MnCHOICES revision in April, DHS has updated or posted forms/documents in the 6791 series. This includes updates to the 6791A, 6791E and 6791F and the new 6791K, 679J and 6791H.

Audience: Lead agency case managers, case management supervisors and people receiving Alternative Care (AC), Essential Community Supports (ECS) and waiver case management

Effective date: Nov. 1, 2023

Summary: Beginning Nov. 1, 2023, lead agencies must meet minimum case management face-to-face requirements for people using the AC program, Brain Injury (BI) Waiver, Community Access for Disability Inclusion (CADI) Waiver, Community Alternative Care (CAC) Waiver, Developmental Disabilities (DD) Waiver, Elderly Waiver (EW) and ECS program. This applies to people whose waiver year ends on or after Nov. 1, 2023.

Audience: Lead agency staff, LTSS providers and other interested parties

Summary: As part of the Own Your Future initiative, DHS will work with FTI Consulting to conduct a study about increasing LTSS access for older adults. Own Your Future aims to help Minnesota prepare for the dramatic increase of people who will be age 85 and older by the year 2030. Many of these older adults will need LTSS.
