Purpose: To announce Moving Home Minnesota’s achievement of moving 1,000 people from institutional to community-based settings.

Since its start in 2013, MHM has helped 1,000 people move out of nursing homes, hospitals, intermediate care facilities for the developmentally disabled and institutions for people with mental diseases.
