The Aging and Adult Services and Disability Services divisions have launched the online Case Manager and Care Coordinator Toolkit, a centralized area for case managers and care coordinators to find DHS resources, training opportunities, policy changes and/or updates, and other resources related to the duties of a case manager and care coordinator..
