The Minnesota Department of Human Services is requesting proposals to establish mobile transition teams throughout the state to facilitate effective transition of children from Psychiatric Residential Treatment Facilities (PRTFs) and Child and Adolescent Behavioral Health Hospitals (CABHH) to community or step-down treatment options.
Children and families experiencing inpatient, psychiatric levels of care need help identifying and coordinating resources for a successful transition to community or step-down treatment options. Many children who no longer meet medical necessity for Psychiatric Residential Treatment Facilities or Child and Adolescent Behavioral Health Hospitals have experienced unnecessarily lengthy stays in these programs, due in part to the lack of available discharge or aftercare resources. This problem has worsened delays in the admissions process for children that imminently require inpatient psychiatric care. Often these children are instead turned away from medically necessary care and forced to look out of state to have their behavioral health needs met.
DHS is seeking proposals for the grant period Aug. 1, 2022 through June 30, 2023.
Proposals submitted in response to this request for proposals must be received at the email address listed in the request no later than 4 p.m., Central Time, on Monday, June 13, 2022. Late proposals will not be considered. Proposals received via other methods will not be considered.
A Responders’ Conference will be held on April 27, 2022, from 2 to 4 p.m. Central Time via Microsoft Teams video conference. The conference will serve as an opportunity for responders to ask specific questions of state staff concerning the project. Attendance at the Responder’s Conference is not mandatory, but is recommended. Details about the Responders’ Conference and how to join can be found in the published request for proposals.
