HCBS community-engagement grants available to reduce gaps for older adults and people with disabilities

DHS invites organizations to apply for community-engagement grants to help improve the delivery of home and community-based services (HCBS) in Minnesota. Selected grantees will lead community research efforts with rural and underserved communities to help inform DHS on HCBS grants and spending initiatives made available through the American Rescue Act. Areas of focus will include ways to improve/expand access for older adults and people with disabilities around:

  • Capacity building for alternative methods of service delivery
  • Technology access
  • Workforce development
  • Caregiver respite
  • Support services.

Details

Awarded grants may range from $5,000 to $50,000. Selected grantees will conduct engagement efforts with specific target populations. Grantees will identify ways to improve inequitable systems and disparities faced by underserved communities who need HCBS services. Grants are awarded through a “quick-call” application, which uses a streamlined submission process.

Target audience

Target audiences for engagement efforts are:

  • American Indian and indigenous people (inclusive of Minnesota sovereign tribal nations)
  • Asian and Pacific Islanders
  • People who are Black and/or African-born
  • People who are of Latin/Hispanic descent
  • LGBTQ communities
  • People who are from rural and regional centers outside the seven-county metro area.
