A qualified residential treatment program (QRTP) assessment by a qualified individual (QI) is required to be completed prior to placement or within 30 days of a placing a child/youth due to crisis in a QRTP certified facility.
A QRTP assessment is not required when placing child/youth in a child care institution (foster residence, group home, and residential treatment program) that is not certified QRTP. These facilities do not meet Title IV-E requirements.
In order for a QI to effectively engage members of a family and permanency team and complete a QRTP assessment, county and Initiative tribal staff are encouraged to work diligently to provide required information to a QI in a timely manner, so an assessment can be completed in required timeline.
Although the estimated time for a QI to complete a QRTP assessment is between 12 to 18 hours, it is generally takes two to three weeks.

When a county or Initiative tribal agency’s juvenile treatment screening team recommends a child/youth be placed in a specific QRTP certified facility, agency staff must verify this specific facility is certified as a QRTP prior to beginning the QRTP assessment process as outlined in Juvenile treatment screening team practice guide, DHS-8120A (PDF) .
Agency staff can review facilities that have been approved for one of the four specialized settings at MN DHS Licensing Info Lookup (state.mn.us).
However, Licensing Look Up does not provide information on which programs within a facility are certified or not. Therefore, the Child Safety and Permanency Division staff are providing this level of detail on specific programs so agency staff know when they place a child in a shelter, evaluation, or intensive residential treatment program if it is certified or not. If an out-of-state facility is being considered, agencies are responsible for verifying QRTP certification by the other state prior to placement.
