Newly Certified BHH Services provider
The Department of Human Services (DHS) welcomes new BHH services provider Metro Behavioral Health, certified November 2021. Metro Behavioral Health has locations in Minneapolis and Moorhead. These additions bring the total number of certified BHH services provider locations to 55.

Fraser’s Behavioral Health Home team serves a primarily pediatric group of patients (ages 2-26) in the Twin Cities/Metro area. Serving children, adolescents and young adults, frequently means that the team is interacting with and supporting the whole family. Often, we see children and families referred for services to support the coordination of care only after a crisis has been identified, such as once a hospitalization has occurred or once a basic need is at risk. Even for families that are enrolled in the program, notification of need doesn’t always come before an emergency has surfaced. As part of the team’s care management strategy, we wanted to see if we could utilize the tools available to us to create a risk stratification tool that would allow us to pro-actively identify individuals and families most at risk, and provide targeted support for those members.
In order to move this strategy forward, Fraser’s Behavioral Health Home team engaged in a “deep dive” of the data available to the program through the EMR registry as well as Minnesota Department of Human Service’s Partner Portal. The data revealed findings indicating that 76% of our patients had two or more diagnosed chronic conditions, 74% of our patients saw 1-4 specialists (some as many as 11), 43% of our patients are on 5 or more prescription medications, and 93% of our patients had a RUB score of a 3 or higher.
To provide a conceptualization of the data in a way that was functional for the team to use, and not get overwhelmed by the amount of information, it was determined that the data element from the Partner Portal that the team would focus on would be the RUB score. A tool was created that incorporates, not just this piece of data from DHS’ Partner Portal, but other measures that have been identified as anticipatory of higher healthcare needs, costs, and utilization. Each month, Fraser’s Integration Specialist identifies patients enrolled who have a RUB score of a 4 or above, as well as any patient whose RUB score has increased in the past 30 days. This information, combined with the other measures, provides a comprehensive risk score to the entire team who then pro-actively intervene and offer an additional layer of support to individuals and families.
The team chose to start small, with only four 4 data elements to make up the risk stratification that the team reviews on a monthly basis. A formal assessment of the impact on individual patients, families and utilization of higher cost interventions will come in the future. However, the team is utilizing the data, as well as our relationship with families, to provide an enhanced level of support and in some cases coordinate before the next crisis arises.

New cohort opportunity
Lung Mind Alliance will start a new cohort in early 2022 for mental health and substance use treatment organizations that want to start integrating tobacco treatment into their existing services. Organizations will receive technical assistance, mentorship, and other resources to support their organizational readiness and process. Learning Days coming April 28, 2022!
Plan now to attend Learning Days, a daylong virtual conference on April 28, 2022. Planning is underway for this annual event where health care home clinics and partners come together to network and learn from each other. We’re excited to once again be working with Showcore, our virtual meeting partner for the 2021 event.
Learning Days 2022 will help participants:
- revisit population health and health equity,
- redesign care delivery to address social determinants of health and strengthen partnerships with community resources,
- recognize the impact of stress and trauma brought on by COVID pandemic, and
- rebuild relationships with community.
The curriculum will include a full day of breakout sessions and workshops, as well as inspiration and resources for self-care in these challenging times. Health care home clinics and community partners will be invited to submit proposals to share their knowledge, skills and experience. The request for proposals went out in December 2021, and registration will open in March 2022. Stay tuned for more information in the coming months!
