The Minnesota Department of Human Services is requesting proposals to provide supportive services for people with serious mental illness who are homeless, long-term homeless, or exiting institutions, and who have complex needs and face high barriers to obtaining and maintaining housing.
Services provided will help people transition to and sustain permanent supportive housing that meets evidence-based practice fidelity standards. Proposals must also address one or more of the behavioral health priorities of program linkage, peer services, veterans, and equity.
The department is seeking proposals for the grant period July 1, 2022, through June 30, 2024.
Proposals submitted in response to this Request for Proposals (RFP) must be received at the email listed in the RFP no later than 4 p.m., Central Time, on March 31, 2022. Late proposals will not be considered. Proposals received via other methods will not be considered.
