Purpose: To announce an upcoming change to the existing long-term care (LTC) service change activity type and new functionality for the developmental disabilities (DD) assessment
Effective: November 2021

With the launch of the revised MnCHOICES application in November 2021, DHS will introduce a functional needs update (FNU) assessment type for both LTC and DD assessments. The FNU assessment type will allow certified assessors to complete remote assessments in certain situations.
