County Notice of Institutions for Mental Disease (IMD) Status

Process update for counties, SUD providers, 1115 Demonstration enrolled providers


The 1115 SUD System Reform Demonstration (“1115 Demonstration”) allows substance use disorder (SUD) services provided within institutions for mental disease (IMDs) to be covered by Medical Assistance (MA). MA enrollees who are in an IMD facility participating in the 1115 Demonstration continue to be eligible for federally funded MA. The Department of Human Services has updated the IMD Status process to reflect these changes.

Please follow the steps below for submitting IMD Status updates:

  1. If a provider is not enrolled in the 1115 Demonstration, skip to step 2. If a provider is enrolled in the 1115 Demonstration, mark “1115 Provider” clearly at the top of the County Notice of IMD Status (DHS-4145) document.
  2. Fax the completed form to the SUD County worker.
  3. SUD County worker updates any service agreement dates.
  4. SUD County worker forwards the County Notice of IMD Status to the County or Tribal Eligibility Worker
  5. The County or Tribal Eligibility Worker reviews the recipient’s file for eligibility for other services making appropriate changes per established Medical Assistance eligibility guidelines.

The County or Tribal Eligibility Worker determines if a change needs to be made and notifies DHS via SIR web form of needed updates. 1115 Demonstration providers do not need to have the major program changed. The 1115 Demonstration federal requirements allow recipients at 1115 Demonstration enrolled IMDs to remain on MA for all federally approved services.

  1. DHS makes changes in the recipient file and replies to the County or Tribal Eligibility Worker via SIR mail.
  2. The County or Tribal Eligibility worker will notify the SUD County worker. The SUD County work approves any pending service agreements.
  3. Once the service agreement is approved, MMIS automatically sends a letter to the provider and the recipient letting them know they can begin billing for the SUD services.
  4. When a recipient is discharged from the IMD facility, the SUD provider must fill out the discharge information on County Notice of IMD Status (DHS-4145) form and follow the same process as above.

If providers have questions about previously submitted County Notice of IMD Status (DHS-4145) forms, please contact the County SUD Worker. The County SUD Worker will coordinate with the County or Tribal Eligibility Worker, who will then work with DHS to make necessary eligibility updates.
