Statewide Efforts

Moving Home Minnesota program extension and change in eligibility requirements

The Moving Home Minnesota program has been extended, with loosened eligibility requirements. These changes help sustain the demonstration project and support more Minnesotans who wish to move from institutions into the community.

On Dec. 27, 2020, the U.S. Congress extended the Money Follows the Person grant (the federal name for Moving Home Minnesota), with additional appropriations in the Consolidated Appropriations Act. The loosened eligibility requirements:

  • Reduce the institutional stay requirement from 90 days to 60 days
  • Include days covered by both Medicare and Medicaid.

The eligibility requirement change is effective immediately. It will allow Moving Home Minnesota to increase the number of transitions and services provided. For more information, see Moving Home Minnesota Program Manual – Eligibility.
