Lead agency review (LAR) begins corrective action
The Lead agency review (LAR) team continues to monitor lead agencies and timelines for providing support plans. Currently, the team is on Round 4 of the statewide review and has not issued corrective actions for the support-plan timeline requirement, as the LAR team was refining protocols and validating data collection.
Effective Jan. 1, 2020, the home and community-based services (HCBS) LAR team began to issue corrective actions when lead agencies do not follow required timelines for providing support plans to people who receive services.

MnCHOICES Support Plan (MnSP) access and navigation modules updated
A new look is on the way for MNSP1000 modules. MnSP access and navigation content is being loaded into the same e-learning software used for MnCAT Step 1 and 2. We updated the modules to include new screenshots, remove launch references and align to current practice. New modules will be launched as they are updated.
Retaking courses will not count toward assessor Continued Learning Units for MnCAT Step 4.
DHS updated exit reason and assessment result codes on LTCC forms
DHS updated exit reason and assessment result codes on long-term care consultation forms to improve the accuracy of data collection. The changes support recently clarified advance notice requirements. Read the full announcement at DHS updated exit reason and assessment result codes on LTCC forms.
Requesting corrections in PCA service agreement start dates
In certain situations, DHS can change the start dates of fee-for-service personal care assistance (PCA) service agreements to avoid gaps in service agreements. For more information, go to Requesting corrections in PCA service agreement start dates.
Activity date versus effective date
It is important to know the difference between activity date and effective date in the long-term care and developmental disabilities screening documents:

Activity date: This is the date the assessment activity actually occurred. Example, if the face-to-face assessment occurred on 7/15/19, the “activity date” on the screening document must be 7/15/19.

Effective date: This is the date the assessment activity becomes effective. This date is on or after the activity date. For example, if the face-to-face assessment occurred on 7/15/19, and the date the outcome of the assessment (such as the start of the waiver span) becomes effective is 8/1/19, the “effective date” on the screening document will be 8/1/19.
Safe At Home participants
Safe at Home is a statewide address confidentiality program designed to help people who fear for their safety maintain a confidential address. Many times program participants are survivors of domestic violence, sexual assault or stalking. When someone enrolls in Safe at Home, they are assigned a PO Box address that they can use as their legal address. In Minnesota, all public and private entities must accept a participant’s assigned address and a participant cannot be required to disclose their real address.
When a lead agency is notified a person is participating in the Safe at Home program, it is important to record the Safe at Home address (PO Box and lot number) instead of the physical location address in the MnCHOICES Assessment and Support Plan.
The person using the Safe at Home program is responsible for disclosure regarding their physical location. Lead agencies must not disclose the physical location to others. That includes disclosure to any service providers.
The person in the Safe at Home program is not required to disclose their physical location address. Assessors working with a person in the Safe at Home program may complete an assessment at a neutral location to maintain privacy if the person does not want to disclose their physical location address.

The Known Issues list is a compilation of items that have been reported to the MnCHOICES team. We created the list to identify issues and provide workaround assistance for common items a user may encounter when working in the assessment or support plan. There are separate Known Issues documents for the assessment and support plan.

MnCHOICES Assessment known issues
Added:
- (NEW 1/17/2020) The home care rating is not recorded in the database after eligibility runs.
- (NEW 1/17/2020) Error message remains in header from selecting assessment with no assessment date even after selecting a different assessment.
- (NEW 1/17/2020) The pediatric symptoms checklist displays through the age of 18 when it should only display through the age of 17.
