MA-ABD applicants and enrollees must sign authorization form to ensure continued MA eligibility

The following notice was sent to lead agency staff, case managers and care coordinators about a newly required authorization form for Medical Assistance enrollees who are 65 or older, have a disability or are blind. The form allows the state and counties to use a new federally required service that identifies unreported accounts when determining eligibility for Medical Assistance.

: To share instructions for lead agency staff to help MA-ABD applicants/enrollees complete and sign an authorization form that will ensure continued MA eligibility

: For questions about people 65 years and older, email dhs.aasd.hcbs@state.mn.us. For questions about people with disabilities who are younger than 65 years, email dsd.responsecenter@state.mn.us

People enrolled in or applying for Medical Assistance for People Who Are Age 65 or Older and People Who Are Blind or Have a Disability (MA-ABD) have begun to receive Account Validation Service (AVS) authorization forms in the mail. Lead agencies should work with applicants/enrollees to make sure they complete this form. If they do not, their MA eligibility will be terminated.
