Account Validation Service (AVS) Requirement for Some Medical Assistance Enrollees

A new form is being sent to Medical Assistance (MA) enrollees who are age 65 or older, blind or who have a disability, and who have an asset limit to qualify for MA. The new form is the “Authorization to Obtain Financial Information from the Account Validation Service (AVS)” (DHS-7823). Federal and state laws require certain MA applicants and enrollees to give the state permission to request financial account information through an account validation service to be eligible for MA. It is important that enrollees complete and return this form to maintain their MA eligibility. They may lose their MA coverage if they do not. This does not apply to children under age 21.

This is a new form that MA enrollees have not received in the past and it is not part of their MA eligibility renewal. DHS is mailing these new forms to MA enrollees in a series of monthly batches through April 2020. Enrollees should wait to receive the form in the mail. The form was sent with a cover letter to the first group of MA enrollees at the end of August. The cover letter explains to enrollees why they are getting the form and what they need to do. MA enrollees who received the form must complete and return it to the county or tribal agency indicated on the cover letter by September 18, 2019. County and tribal agencies will send an advance 10-day closing notice to enrollees who do not complete and return the form by September 18, 2019. If enrollees return the completed form by September 30, 2019, their MA coverage will continue.  If they do not, their coverage will end starting October 1, 2019. If they need more time or help to complete the form, or they have questions on the form, they should contact their county or tribal agency as soon as possible but no later than September 30, 2019, and the agency will continue their MA coverage for October.
