DHS sees Certified Community Behavioral Health Clinics (CCBHC) as an important piece for the future of mental health and substance use disorder care in Minnesota. Like other states, we are working to move past the demonstration project and find a path forward to continue this service beyond the federal demonstration period (ending June 30, 2019). Please continue to manage CCBHC as you have for the last 2 years until further notice from DHS.
Background
CCBHC is a federal demonstration project that has been running from July 1, 2017 through June 30, 2019 with 6 clinics in MN participating. Not only does the demonstration provide an opportunity to demonstrate an integrated behavioral health (outpatient mental health and substance use disorder treatment services) clinic model but also a new payment model. Each of the 6 demonstration clinics receives a Prospective Payment System (PPS) wrap payment. These 6 clinics are:
- Northern Pines Mental Health Center (Brainerd)
- Northwestern Mental Health Center (Crookston)
- People Incorporated (4 Twin Cities Metro Counties)
- Wilder Mental Health and Wellness (St. Paul)
- Ramsey County Mental Health Center (St. Paul)
- Zumbro Valley Health Center (Rochester)
Beginning in October of 2018, 2 additional clinics received SAMHSA grant funds to provide the CCBHC model. These clinics are funded through SAMHSA grant funds and are not covered by the MN CCBHC statute, do not follow the implementation guidance, and do not receive the Prospective Payment (PPS) rate from Medical Assistance. These 2 clinics are:
- Western Mental Health Center (Marshall)
- Human Development Center (Duluth)
In order to provide the CCBHC model in MN, based on federal CCBHC criteria, DHS had to employ a number of tools to avoid conflict or duplication between MN statute, rule or standards and the CCBHC criteria.
Current Update
DHS is applying for federal approval to continue billing Medicaid for this model. DHS and the CCBHC providers are also proposing legislative changes to the CCBHC statute to continue the model and payment. Pending approval of those items, please continue the following:
- Legislative changes being proposed include planning for the SAMHSA grant funded clinics to be included and for the model to go statewide in the future
- All items listed above (bulletins, statute, implementation guidance) will remain valid until state legislative and CMS approval is received
- Upon state legislative and CMS approval, the bulletins and implementation guidance will be updated to reflect any changes
- There will continue to be no county share when Medical Assistance pays for targeted case management and substance use disorder treatment services except for the 2 SAMHSA grant funded clinics
- CCBHC providers will continue to have direct access to outpatient substance use disorder treatment for Medicaid fee for service clients within their own clinics through using CHATS until providers are informed otherwise
