The annual Learning Days conference brings together more than 300 health care providers, public health professionals and community partners to exchange information, advance health transformation, and build relationships.

There will be a workshop for BHH services providers on April 9 from 1:00 p.m. - 4:30 p.m., during the afternoon pre-conference. Information about how to register for the workshop was sent to the BHH services providers authorized representatives via email on January 31, 2019 and is separate from the conference registration.
You are invited to the third webinar in the Population Health Management series
When: Tuesday, February 26, 2019
Time: 11am – 12pm
This webinar will feature the work of two BHH providers and their journey into population health management. The discussion will include how to identify specific populations to focus on and gather detailed information to provide more intensive interventions. It will also include how to use the patient registry to track preventative screenings to more easily identify care gaps, implement strategies to improve patient care and outcomes through education, and increase referrals to primary care and other community partners.
