DHS Announces New Form DHS-6633A Community Support Plan Addendum and Updates DHS-3965 Elderly Waiver Conversion Rate Request

CDCS Community Support Plan Addendum with 2018 EW and AC Rate Increase is
now available. This form is used to notify EW and AC CDCS participants of
the .03% increase in their CDCS budgets effective July 1,
2018 that was a result of the rate increase
for the home delivered meals service on the EW/AC programs.
